Step to Home Program
The Step to Home Program is designed for HCF skilled nursing facilities that also include assisted living, as well as for our standalone assisted living communities. This program facilitates the transition of short-term skilled patients who are nearing discharge from a rehabilitation stay at a skilled nursing facility to an assisted living community, rather than directly to their home.
While any patient suitable for assisted living care can transition from a skilled nursing facility to an assisted living community, the Step to Home Program specifically targets patients who will require post-SNF home health care services at the assisted living for a short-term period, aiding in their transition from skilled nursing facility care.
Under this program, a home care agency will deliver skilled services to the patient at the assisted living community, where they will be charged the designated discounted "Step to Home Program" rate for room and board. This initiative aims to provide a supportive environment for recovery and rehabilitation, bridging the gap between skilled nursing facility care and eventual return home.
Receiving post-SNF home care services in an assisted living environment offers several benefits to the patient. One advantage is that their progress can be closely monitored, which may not be as feasible if they were discharged directly to their own home. This closer monitoring ensures that their recovery and rehabilitation are effectively managed, with healthcare professionals readily available to provide support and assistance as needed.
Additionally, transitioning to an assisted living environment allows the patient to work on gaining independence in a setting that offers more autonomy compared to a skilled nursing facility. This environment encourages self-care and daily activities while still providing necessary medical oversight and support.
